Marisol Vega
Intake Summary / First appointment — 26 Nov 2025
Opening rapport notes:
Marisol arrived a few minutes early and spoke quickly for the first ten minutes, then settled. She said she had rehearsed what to say and apologised twice for "taking up the slot".
Presenting concerns (client's own words):
Reports a flat, wrung-out feeling since roughly late August.
Describes carrying "a wet coat all day" that never dries out.
States "I keep waiting for the part of me that used to enjoy things".
Emotional & physical review:
Low mood persisting most days for about fourteen weeks, with reduced interest in ceramics, choir and her balcony herbs. Physical complaints include chest tightness, a racing pulse in the evenings, shallow breathing on the walk to work, trembling hands and a heavy fatigue by mid-afternoon. Episodes cluster around 4–6pm. Sleep onset takes 90 minutes or more; wakes around 3am and rarely returns to sleep. Appetite reduced — skips two meals most weekdays, largely through forgetting. Two episodes of unexpected crying this month, both while driving home.
Brief life context:
Works as a logistics coordinator and describes the role as "fine, just loud". Tends a shared balcony garden, currently neglected. Close friends have offered to help and Marisol declines, citing embarrassment. Family live six hours away and contact has thinned since spring; she has not yet told them.
Risk screen:
No current intent, plan or means. Two fleeting passive thoughts in October, both described as "wanting the noise to stop", neither acted on. Protective factors: her dog, the Wednesday choir, and a supportive line manager who has already agreed to shorter shifts. Agreed to call the practice line rather than wait for the next appointment if the 3am waking worsens.
