Marisol Ferreira
Intake & Formulation / Structured Interview
Opening rapport notes:
Marisol arrived a few minutes early and apologised twice for taking the
appointment. Guarded at first, warmer once the recording was explained.
Said a colleague had pushed her to book.
Presenting concerns (client's own words):
Describes seven weeks of "running on a flat battery". Reports waking
between 3 and 4 AM most nights and being unable to settle again. Says she
has stopped answering her sister's calls because "there's nothing new to
report and I hate the pity".
Emotional & physical symptom check:
Persistent worry present most days for roughly four months, described as
"a hum I can't switch off". Four episodes of acute anxiety in the past
three weeks — chest tightness, shallow breathing, hands going numb,
lasting eight to twenty minutes and followed by several hours of fatigue.
Triggers reported as unopened email and unexpected phone calls. Sleep
onset delayed by 60–90 minutes; early waking as above. Appetite reduced,
skipping lunch on most working days, weight down an estimated 4 kg since
August. Concentration noticeably poorer — re-reads the same paragraph
several times before it registers. No alcohol increase reported; caffeine
up to five cups daily.
Brief life context:
Works as a logistics coordinator on a rotating shift pattern and has taken
on a second team since a colleague's departure. Lives alone in a
first-floor flat with a small balcony she has stopped watering. No current
partner. Close friends nearby but declining most invitations. Family are
overseas and contact has thinned to short messages; has not yet told them
about the referral.
