Marisol Ferrand
Intake Assessment / Structured Interview Template
Opening Rapport Notes:
Marisol presented as guarded and visibly self-critical during the first minutes of contact. Thanked the clinician for the appointment while noting how long she had put off asking for support.
Presenting Concerns (in client's own words):
Reports being "wound up from the moment I wake" for most of the past half year, with recent night-time episodes she found alarming. Describes carrying a constant weight and having stopped things she used to look forward to. States "I can't switch my head off" and "everything feels like it's due at once."
Emotional & Physical Symptom Check:
Persistent low mood and tension across roughly 6 months. Marked anhedonia — has withdrawn from choir practice, swimming, and tending her balcony plants. Four acute episodes in the past month involving chest tightness, racing heart and shortness of breath, lasting 8–20 minutes with several hours of trembling and depletion afterwards. Episodes typically follow workload spikes or unexpected schedule changes. Sleep onset takes 60–90 minutes; consistent waking at 3–4 AM with no return to sleep. Appetite noticeably reduced — one substantial meal per day, other meals skipped through forgetting or lack of drive. Ongoing fatigue and flattened energy. Tearful episodes concentrated in the early morning.
Brief Life Context:
Works as a logistics coordinator and reports slipping performance over the last quarter. Lives alone in a first-floor flat with a small balcony (currently unattended). No current partner. Close friends have tried to check in, but Marisol tends to deflect out of embarrassment. Family are several hours away and contact has thinned; she has not yet told them any of this.
